Contraindications: why not take Buspar?

CONTRA-INDICATED:

  • – Known hypersensitivity to any component of the drug.
  • – Severe renal and / or hepatic insufficiency.
  • – Due to the presence of lactose , this medication is contraindicated in cases of congenital galactosemia, glucose and galactose malabsorption syndrome or lactase deficiency.

ADVISED AGAINST:

  • – Buspirone is not recommended in patients with a history of seizures.
  • – This medication is not recommended in children and adolescents under 18 years of age, in the absence of data.
  • – The use of this medication is generally not recommended during pregnancy, whatever the term: clinical and animal data are insufficient and do not allow a conclusion. In view of the available data, the use of buspirone is not recommended during pregnancy, whatever the term, and in women of childbearing potential not using contraceptive measures.
  • – Breast-feeding: in the absence of data on the passage into breast milk, the risk is not known, therefore, breast-feeding is not recommended during the treatment.
  • – Combinations not recommended : erythromycin, itraconazole.

Posology and method of administration

  •  The treatment will be initiated on the basis of 1/2 tablet 3 times a day.
  • Thereafter, the dosage will be adjusted according to the patient’s needs and the daily dose may be increased in steps of 5 mg, every two or three days depending on the therapeutic response.
  • – The daily dose should usually be between 15 and 20 mg evenly divided during the day, in 2 or 3 divided doses.
  • – The maximum dose of 60 mg per day will not be exceeded.
  • – No dose adjustment is required in the elderly.
  • – In case of substitution treatment with benzodiazepines :
  • . Buspirone should be introduced 15 days before the start of benzodiazepine withdrawal.
  • . The dosage of the benzodiazepine should be gradually reduced while strictly observing the usual rules for this withdrawal (buspirone does not prevent or treat the benzodiazepine withdrawal syndrome).

How to stop treatment :

Stopping treatment with buspirone may be rapid.

Duration of treatment :

  •  Reactive anxiety, adjunct treatment for anxiety during neuroses, anxiety associated with severe or painful somatic illness: treatment should not exceed 4 to 6 weeks.
  • – Generalized anxiety: long-term treatment after consulting a specialist.

Warnings and precautions for use Buspar

WARNINGS:

PRECAUTIONS FOR USE:

  •  This medication is not recommended for children and adolescents under 18, in the absence of data.

  • – Rebound, physical and psychological drug dependence :
  • in specific studies, no dependence potential has been described.

Depression

Buspirone acts primarily on the anxiety component of depression. Used alone, it is not a treatment for depression and may possibly mask the signs.

  •  Pregnancy: clinical and animal data are insufficient and do not allow a conclusion. In view of the available data, the use of buspirone is not recommended during pregnancy, whatever the term, and in women of childbearing potential not using contraceptive measures.
  • – Combinations requiring precautions for use : diltiazem, verapamil; rifampicin.

Interactions with other drugs and other forms of interactions

RECOMMENDED ASSOCIATIONS :

  • Erythromycin :
    Increase in plasma concentrations of the anxiolytic by decrease in its hepatic metabolism, with significant increase in sedation.
  • Itraconazole :
    Increase in plasma concentrations of the anxiolytic by reduction of its hepatic metabolism, with significant increase in sedation.

ASSOCIATIONS REQUIRING PRECAUTIONS FOR USE :

– Diltiazem, verapamil :
Increase in the plasma concentrations of buspirone by decrease in its hepatic metabolism, with increase in its adverse effects.
Clinical monitoring and adjustment of the dosage of buspirone if necessary.
– Rifampicin :
Decrease in the plasma concentrations of buspirone by increasing its hepatic metabolism by rifampicin.
Clinical monitoring and possible adjustment of the dosage of buspirone during treatment with rifampicin and after its discontinuation.

COMBINATIONS TO BE TAKEN INTO ACCOUNT :

– Diazepam :
Risk of increased adverse effects of buspirone.
– Grapefruit juice :
Risk of increased adverse effects of buspirone by reduction of its metabolism by grapefruit juice.

Side effects of Buspar

  • – Dizziness or drunkenness, headache, nausea, nervousness, wetness or sweating, gastralgia.
  • – These side effects are generally observed at the start of treatment and usually decrease or disappear with the continuation of the treatment and / or the reduction of the dosage.
  • – Other rare side effects have been reported since marketing (<1/10000)
  • . skin reactions such as rash and hives
  • . neurological disorders such as seizures and serotonin syndrome
  • . priapism.

Overdose

In the event of an overdose, after gastric evacuation, symptomatic treatment should be instituted if necessary. There is no specific antidote. Buspirone is not hemodialysable.

Effect on ability to drive and use machines

Attention is drawn, particularly among vehicle drivers and machine users, to the risks of dizziness or drunkenness associated with the use of this medication.
